Please contact us to discuss a possible placement or for more information

Process of Application:

After a genuine referral is received, the assessment process is initiated and comprises of the following.

Assessment and Behavioural Analysis:

The manager or the proprietor will visit the client and parents/carers in the home or their residential placement to complete an in-depth assessment of the applicant. It can also be very helpful if parents of prospective service users can complete this form to enable the assessor to understand the needs and abilities of the client.

Suitability Meeting:

The manager, the proprietor of the home and the social worker will meet to discuss the suitability of the placement.

Visit to the appropriate service:

The applicant, their parents/guardians and social worker are invited to No. 76 Canute Road, to view the home and talk about the service that we shall provide if the application is successful.

A decision will be made as soon as possible and once funding is in place, we will inform all relevant persons involved.

Transition Meeting:

All relevant persons will meet to discuss and develop a transition that will minimise the anxieties that the service user will experience at this time.

On Admission:

The service user will be assigned a keyworker who is responsible for continuing the ongoing assessment, planning, implementation and evaluation of care which is needed for the individual. This will be reviewed at a planning meeting arranged at a time that is suitable to all parties involved., usually 46 weeks after admission
